1. Instructions:

(a) Each parent must individually complete and submit the referral form below prior to commencing the intake process.

(b) You will be asked to upload a copy of the front of your drivers license or the identification page from your current passport. Please ensure you have this document ready in either PDF or JPG/JPEG format.

(c) You will be asked to upload a copy of various legal documents including (where applicable) Parenting Orders, Consent Orders, Parenting Plans and AVOs. Please ensure you have these documents in PDF or JPG/JPEG format.

(d) It is not the responsibility of the service to contact or induce a parent to complete this form. The Service recommends that enquiries be made with your legal representative should you be concerned that the other parent is unwilling to submit a referral form to us.  

(e) The Service Manager will only make contact with both parents to commence the intake process once both parents have satisfactorily completed  and submitted this referral form. 

2. Terms and Conditions:

In completing and submitting this form, you understand and accept that:

(a) A Family File will be created on submission of both parent’s referral forms.

(b) Inaccurate or incomplete information provided in the referral form may delay the commencement of service entry or exclude the family from service entry.

(c) Information and attachments provided in the referral form will be included in the family file.

(d) Assessment of families for suitability for the Service is at the discretion of the Service Manager.  

(e) In the event that the Service assesses a family as unsuitable for the Service, both parents will be notified in writing and a notation will be entered into the Family File. 

(f) The Service reserves the right to deny access to its services without explanation, however parents may subpoena the Family File
